Supported Living
Our model is to provide personal care in ways that are tailored to suit the needs of the individual, focusing on a high quality of care and continuous improvement of their health status. We provide support to people who still live in their own homes but who require additional support with household tasks, personal care or any other activity that allows them to maintain their independence and quality of life.
Supported Living services offered:
- Person-centred support
- Improve the care and support that people get
- We ensure that people are at the heart of everything we do
- Maintaining a level of care and support that is the same quality wherever they live
- Our services meet the standards set out by the Care Quality Commission.
Educational and Skills Support
One of our objectives is to work with employment and education providers to ensure the people we support are able to access paid and unpaid employment, and also gain access to educational training by providing learning opportunities which will help build their confidence.

Community-minded
We will create strong links within community pathways, specifically with regards to special educational needs, for providing training and development to people with disabilities empowering them to choose further integration and participation within the wider community.

Social activities
We will build good relationships with the local day centre to offer services which enable the individual to engage in social activities which will be tailored to their abilities and health conditions.
These could be arts and crafts, mental stimulation games such as bingo, gentle exercise, discussion groups (for books, films, or current events, for example), holiday and birthday celebrations, local outings, or other activities of their choice.
This will help them live a fulfilling life within the community and beyond.
REACH Standards:
It is about the human rights of all, and it is our duty as a healthcare provider to ensure the REACH standards are met.
- I choose my friends and my relationship
- I choose how I am part of the community
- I get help to make changes in my life
- Choose who supports them and how they are supported
- Choose how to be healthy and safe
- The person chooses who they live with and where he/she lives and
- has their own home with a tenancy/ownership
- Choose how they can be part of the community and have the same rights and responsibilities as other citizens

Housing Support:
Although the care package is separated from the accommodation contract agreement, we work together with the housing association to source accommodation that fits the need of the person.
As this provides assurance and value for money in terms of repair and good management of their homes.
We ensure the tenancy agreement gives certain rights to both us and the landlord.
The agreement will contain details of the landlord’s obligation to repair the property.
For many people who want to live independently, getting the right care and support is just as important as finding the right home.

Money and Bills Advice:
We encourage those in our care to remain in control of their money and financial affairs. As this is important for retaining their independence and personal dignity.

In addition, we will provide support with the following:
- Benefits claims and advice
- Budgeting when required
- Maximising your income
- Managing daily living bills of food and personal expenses
- Understanding household bills
- Support with day-to-day spending
Safe and Supportive Environment:
We prioritise the safety and comfort of our service users. Our living spaces are designed to be accessible and accommodating, and we maintain a secure environment that fosters a sense of community and belonging.
Engaging Activities and Programs:
We believe in enriching the lives of our service users through meaningful activities and programs. We provide a range of opportunities for individuals to engage, learn, and connect.
